Relationship to Property Owner
Select the description from the drop down menu that most accurately describes the person for whom this HMI Horticultural Assessment is being prepared and his or her relationship to the owner of the assessed property.

Percentage Of Property Landscaped
This is an optional field. You can estimate the percentage of the total property landscaped. Do not include grassy or naturalized areas in your estimate.

Irrigation Restrictions
Enter “yes” if your local government currently restricts water usage. For example, if you can only water your lawn on Tuesdays and Thursdays, you will select “yes”.

Soil Type
This is an optional field. If you do not know the dominant soil type on this property, leave the field blank.

Landscape Maintenance Company
Enter the name of the company you use for maintaining the landscape on this property, if any.

Tree Care Company
Enter the name of the company that you use for maintaining the trees and shrubs on this property, if any.

Genus Search
Select the plant’s genus from the drop down menu. After selecting the genus, select the species in the next box.

Species Search
Select the plant’s species from the drop down menu. After selecting the species, select the plant from the Plant Search box below.

Plant Search
Select either the genus species (i.e. Cercis canadensis) or genus, species and cultivar (i.e., Cercis canadensis ‘Forest Pansy’) record.

Name Search
Use this feature to search for a plant by its scientific name, common name or cultivar. Options are displayed in the Plant Search box below.

QTY
For trees, the quantity will ALWAYS be 1 (one). For shrubs, which are identical (type, size, location, health, etc.), the quantity can be greater than 1 (one).

Height/Spread
Height/Spread is measured in feet and is required for all plants assessed. Enter the larger of the two. For example, if the plant is 3 feet tall and 5 feet wide, enter 5 feet. .

Caliper/DBH
Cal/Diameter at Breast Height (DBH) is required for all trees assessed. The diameter (not circumference) of the trunk is taken roughly 4½ feet from the ground on each trunk stem, which will typically be 1 (one). If there are multiple stems, enter the measurement of each stem. To do this, select “Multi” from the Cal/DBH drop down menu. This will open a Multi-Stem Entry box below the Cal/DBH box.

Multi-Stem Entry
Select the size of the first stem from the drop down menu and then press the ADD button. Repeat this process for each stem. You can remove an entry by pressing the Remove button. You can change an entry by pressing the Change button, selecting the correct size from the drop down menu, and then pressing the CHANGE button again.
